The checklist below provides a guide for planning your wedding at St. Alphonsus.
8-12 Months Prior to Wedding
Once your wedding date is confirmed, you will meet with a wedding ministry coordinator or pastor to review the necessary steps and documents to prepare for your wedding.
Immediately following your wedding preparation meeting, you will register for the One in Christ marriage seminar (payment for the seminar is made at the time of online registration)
6 Months Prior to Wedding
Complete the FOCCUS online survey
FOCCUS survey links will be sent to the email address you provided when requesting your wedding date; access these links to complete the FOCCUS survey. If 90 days have passed since the links were emailed to you, they will need to be reset. Contact our parish office if you need to have your FOCCUS links reset.
Order and submit baptism and confirmation certificates
The document must be a hard copy with an evident church seal or stamp issued and dated within six months of your wedding date. No copies or scanned emails are permitted. They can be sent to or dropped off at St. Alphonsus and will become part of your permanent wedding file.
3 Months Prior to Wedding
Expect a FOCCUS Facilitating Couple to contact you to review your FOCCUS survey results
2 Months Prior to Wedding
A St. Alphonsus priest will contact you to arrange a meeting to complete the Pre-nuptial Questionnaire
Expect a Wedding Coordinator to contact you to confirm your ceremony planning form
Your ceremony planning form will be emailed to you once your wedding date is confirmed.
1 Month Prior to Wedding
Secure a marriage license and bring the original to St. Alphonsus at least one week prior to the wedding.
Please keep a copy of your marriage license for yourself. On the day of your wedding, the priest will sign it and we will mail it to Cook County Vital Records.
